The health, safety, and wellbeing of everyone within Grace Healthcare – YCYW workplaces, including employees, contractors, visitors, the public, neighbors, and the community, is our highest priority. As a result of the COVID-19 outbreak, Grace Healthcare – YCYW is applying various processes to safeguard everyone in our workplaces. This Questionnaire must be completed by all persons upon entry to the site. Grace Healthcare – YCYW and the subcontractors must ensure that any transient workers, consultants, or other visitors that arrive on the site during the day also complete this document. Note: Close Contact means having face-to-face contact for more than 15 minutes with someone who has a confirmed case of coronavirus (COVID-19) – or sharing a closed space with them for more than two hours.
